To qualify, you must:

You must be enrolled with Blue Cross and Blue Shield of Minnesota and Blue Plus
Pregnant and in your 20-27 week of pregnancy

You also must not fall under any of the following exclusion criteria:

Chronic Kidney Disease Stage 5 or End Stage Renal Disease
Live in hospice, a skilled nursing home, a long-term care facility, or get meals from another program.
Have cancer and are getting chemotherapy or are in palliative care.
Liquid diet
Life-threatening food allergy (may join with consent).

You’ll need your health plan ID card to confirm eligibility.

Is there any cost to me?

What information am I required to provide to request food deliveries?

What food will I receive and how often?

Once finish your enrollment, your first food delivery will arrive at your home. Delivery usually takes 3– 10 business days. You can choose between the following options and switch at any time based on your needs during pregnancy or after:

Prepared meals — ready to heat and eat, delivered about every 4 weeks (each delivery includes 10 prepared meals)
Groceries — boxes with foods like fruits, vegetables, pantry items, meat, and dairy, delivered about every 2 weeks

You will receive updates by phone, text, or email based on your preference.

How does nutrition coaching work?

After you join, you’ll schedule your first nutrition coaching consult. We’ll contact you with a link or instructions based on your preferred method (text, email, or phone), or you can call us and we’ll schedule it for you.

Your nutrition coaching sessions are 20-minute phone calls with a NourishedRx nutrition coach. You’ll have up to 6 consults over 6 months. We’ll remind you to schedule a consult, but you can schedule a session at any time if you need extra support.

A NourishedRx wellness team member will also be available to help you along the way if you have questions.
