To qualify, you must:

Be enrolled with BlueCross BlueShield of South Carolina
Be diagnosed with type 2 diabetes

You must also not meet any of the following exclusion criteria:

Following a liquid-only diet
Diagnosed with stage 5 chronic kidney disease, end-stage renal disease, or receiving dialysis
Enrolled in hospice care
Living in a nursing home or long-term care facility
Other exclusion criteria may apply
