NourishedRx will verify the ID for members who received invitations to participate. If you did not receive an invitation, we will not be able to recognize the ID number provided.

and do not meet any of the following exclusion criteria: liquid diet, stage 5 chronic kidney disease, end-stage renal disease, renal replacement or dialysis, enrolled in hospice, or living in a nursing or long term care facility. Members with Medicare Part B or Medicare Advantage as Primary policy are ineligible.

As part of the enrollment process, you will be required to provide your name, date of birth, health plan Member ID available from your Blue Cross and Blue Shield Federal Employee Program Member ID card, contact information, and you may be required to provide a copy of your most recent HbA1c lab result (taken in the last 6 months).

The program is intended to support members with uncontrolled diabetes. The HbA1c indicates the average of blood sugar values over a three month period. HbA1c values of 8 and greater are considered uncontrolled. To participate, you may be required to upload an HbA1c lab result taken in the last six months, and have another value taken approximately four months after program enrollment to help assess the impact of the program

After we’ve confirmed your eligibility, you’ll complete an intake assessment and schedule your first nutrition coaching session with a NourishedRx registered dietitian. You will receive your first healthy home-delivered food after completing your intake assessment, and will continue to receive food for 4 months. You will engage in nutrition coaching sessions with a NourishedRx registered dietitian and you will have additional support from a NourishedRx wellness associate for 6 months.

You will meet with the registered dietitian telephonically on an ongoing basis over a 6 month period. The registered dietitian will work with you to understand your health, motivations, and challenges, and take you through an education curriculum tailored to your needs and goals. The registered dietitian will facilitate balanced and nutrient-rich home-delivered food on your behalf, accounting for your dietary restrictions, food preferences, and feedback on previous deliveries.

Your wellness associate will help you with enrollment and continued engagement in the program, answer any
questions you might have about the program, encourage and motivate you throughout the program, and help connect you to Blue Cross and Blue Shield Federal Employee Program or community-based resources that might be helpful to you.

After you enroll in the program, you will receive food for 4 months, receive support from a dietitian and wellness associate for 6 months, and have access to online resources for up to 12 months. Please note that this program can change or be eliminated at any time.

No - you are eligible to participate one time.

You will receive a combination of healthy prepared meals, meal kits, and grocery bundles based on your selection. Groceries may include fresh produce, shelf-stable pantry items, meat, and dairy.You’ll receive a variety of food support over 12 weeks, based on your preferences and needs:

• Weeks 1–3: Fully prepared meals + a pantry kit delivered to your home (30 meals total)
• Weeks 4–5: Meal kits delivered to help you cook at home (12 servings total)
• Weeks 6–16: Weekly or biweekly grocery and produce boxes, tailored to your dietary needs

During Weeks 1–3, you’ll receive a total of 3 food box deliveries of 10 prepared meals. From Weeks 4–5, you’ll get 2 meal kit deliveries (12 total servings). From Weeks 6–16, you’ll receive bi-weekly grocery deliveries.
