Welcome to Nourishing Wake!

Enrollment in this program is now at capacity. Thank you for your interest in Nourishing Wake.

To be eligible for this benefit, you must be:

• A resident of Wake County
• Be 18 or older
• Referred by Advance Community Health, Urban Ministries of Wake County, WakeMed, or Duke Health. If you're not eligible at this time, but see a physician at one of these clinics, please contact your health center for a referral to Nourishing Wake.

To verify your eligibility, you will be required to provide your name, date of birth, and ZIP code.

Once you get started with the benefit, you will complete a quick assessment. After that, the NourishedRx team will place your first food order. You will be contacted about your food deliveries by email, text, or phone, based on your preferences.

You will receive a combination of fresh produce and proteins, all sourced locally and from Farmer Foodshare.

You will receive your first healthy home-delivered food after completing your intake assessment. You will receive order and delivery notifications for each food order.

No — you can provide input and feedback on the food you receive, but what’s in season will determine what’s available in your food delivery.

NourishedRx partners with Farmer Foodshare, who sources from North Carolina farms.

You will have an opportunity to rate the food you receive throughout the program through surveys.

You will receive food order and delivery notifications through email, text, phone - whichever you prefer.
