How do I get started?

How long does the program last?

The program lasts 12 weeks. During that time, you’ll receive food deliveries and have access to a nutrition coach and educational resources to support your health goals.

How do I know if I meet the requirements to get food deliveries?

To qualify, you must:

  • Be enrolled in a Medicare Advantage plan under the Advantage Plus Network in Connecticut (Anthem, UnitedHealthcare, or CarePartners of Connecticut)
  • Diagnosed with Type 2 Diabetes (T2D) OR Type 1 Diabetes
  • Have an A1C of 9.0 or higher within the past 12 months

You must also not meet any of the following exclusion criteria:

  • Chronic Kidney Disease Stage 5 or End Stage Renal Disease
  • Hospice, skilled nursing facility, long-term care facility, or other supportive meal plan
  • Cancer diagnosis with ongoing chemotherapy and/or flagged for palliative care
  • Liquid diet
  • Life-threatening food allergy (may participate with consent)
  • Insufficient facilities for food preparation, unable to prepare food, or homeless*

Is there any cost to me?

No, there is no cost to you. This program is fully covered as part of your Advantage Plus Network benefits — no copays, no fees, and no surprise bills.

What information am I required to provide to request food deliveries?

To verify your eligibility, you will be required to provide your name, date of birth and health plan Member ID. You will also be required to provide valid contact information.

What can I expect after signing up?

Once you get started, you’ll complete a quick survey and then your first healthy food delivery will be on its way. You’ll also have the option to schedule a consult with a NourishedRx nutrition coach for personalized support, and you’ll have ongoing access to online education and nutrition resources. As a participant, we’ll also send you a few short surveys and check-ins to hear how you’re doing and get feedback on your experience.

What types of food will I receive?

You will receive healthy grocery boxes delivered to your home. These may include fresh fruits and vegetables, pantry staples, proteins, and dairy—tailored to your dietary needs and preferences.

How often can I receive food deliveries?

You will receive food order and delivery notifications through email, text, phone - whichever you prefer. We recommend providing your email to make sure you get updates quickly and easily.

When will I receive the food?

You will receive your first healthy home-delivered food after completing your intake assessment. You will receive order and delivery notifications for each food order.

Can I order my own food?

No, but you’ll be able to share your input and feedback on the food you receive, and your nutrition coach will use that to carefully select balanced, nutrient-rich meals on your behalf.

Where does the food come from?

NourishedRx partners with a variety of food suppliers, including national and local suppliers, to deliver healthy meals and groceries.

What if I don’t like the food?

You will have an opportunity to rate the food you receive throughout the program through surveys, and you can also share feedback with your nutrition coach and wellness associate. The coach you work with will incorporate your feedback into future food deliveries to best meet your needs and preferences.

How will I know when the food will be delivered?

You will receive food order and delivery notifications through email, text, phone - whichever you prefer. We recommend providing your email to make sure you get updates quickly and easily.

Can I pause the program if something comes up?

Yes, if you need to pause the program, please reach out to us as soon as possible. Our wellness associates are happy to help coordinate any adjustments. Please note that depending on timing, some orders may already be in progress and unable to be canceled.
