The program lasts 6 months. During that time, you’ll receive a Fresh Connect card to buy fresh fruits and vegetables to support your health goals. After 5 months, you will receive a call from the NourishedRx team to see if you are eligible to extend to another 6 months.

No, there is no cost to you. This program is fully covered by your health plan — no copays, no fees, and no surprise bills.

Once you confirm your address, your Fresh Connect Produce+ card will be sent in the mail and the Fresh Connect team will reach out in 2-3 weeks to confirm you received your card. You can use your Fresh Connect card to buy fresh, frozen, and canned fruits and vegetables at eligible grocery stores nationwide. After you fill out the brief survey, we will confirm your eligibility and send you your Fresh Connect card in the mail. The Fresh Connect team will reach out in 2-3 weeks to confirm you received your card and to provide more program details.

No. Your $100 monthly amount does not roll over — any unused dollars expire on the last day of the month and start fresh on the 1st. Shop early to make the most of every dollar.

No. At this time, Fresh Connect cards are not accepted online and cannot be used for online pickup or delivery orders.

No, programs cannot be paused.  If you have an issue (e.g. become hospitalized or cannot participate in the program for any reason), please contact your care manager.

No. Fresh Connect is included with your health plan and isn't recorded as income. It also doesn't affect SNAP, WIC, or any other government program you participate in.
