The program lasts 6 months total.

You must also not meet any of the following exclusion criteria:

• Chronic Kidney Disease Stage 5 or End Stage Renal Disease
• Enrolled in hospice, skilled nursing facility, long-term care facility, or other supportive meal plan
• Have cancer and are getting chemotherapy or are in palliative care
• Be on liquid diet
• Life-threatening food allergy (may participate with consent)
• No kitchen, cannot cook, or are homeless.

Other exclusion criteria may apply

No, there is no cost to you. This program provides complete coverage for eligible patients at People’s Center — no copays, no fees, and no surprise bills.

To see if you qualify, you will provide some basic information. We need your name and date of birth. We'll also need your contact information.

• Prepared Meals — ready to heat and eat
• Groceries — boxes of food. They can include fruit and vegetables. They can also include pantry items, meat, and dairy

Your food is selected to support your health needs and preferences.

You will get regular food deliveries. They will come during the program, while you are pregnant, and after you give birth.

After you finish the intake, we will deliver your first healthy meal to your home. You will get order and delivery notices for each delivery.

No, but you can share feedback on the food you get. Your nutrition coach will use it to pick healthy, balanced choices for you.

We work with national and local food suppliers to deliver healthy groceries.

Tell us what you think about your food in quick surveys. You can also share feedback with your coach or your wellness associate. Your coach will use your feedback to help improve future food deliveries.

We'll send order and delivery notifications by email, text, or phone—whichever you choose. Add your email so you get updates fast.

Yes, if you need to pause the program, please reach out to us as soon as possible. Our wellness associates can help with adjustments. Please note that some orders might already be in progress due to timing; so, we cannot cancel them.
